Patients are responsible for making sure that Dr. Stuart Hetrick is in-network with their insurance provider. We verify benefits and eligibility, but we do not verify that we are in each individual network.
As a patient, it is in your best interest to know if your insurance plan is contracted with Foothills Neurology, and to understand your insurance plan benefits and also your responsibility for any deductibles, co-insurance, or co-payment amounts prior to any visit. You may have different deductibles, co-insurance, or co-payment amounts, depending on the contracted status of your insurance company.
It is also important to understand your insurance plan’s current benefit and coverage rules. Policies and coverage determinations may vary from year to year.
I understand I am financially responsible for any co-payments, deductibles, coinsurance and all charges, which are not covered by my insurance. I understand that verification of coverage is not a guarantee of payment of benefits. Your insurance company determines insurance benefit payments. I understand I will be responsible for the portion not covered by my insurance.
I understand a $50 charge will be made for broken appointments unless 24-hour notice is given.
